Adoption Form

Congratulations on your decision to welcome a new family member! And thank you for choosing HOPE to help find your fur-ever friend.

By filling out this application, it will expedite the adoption process and will also assist our adoption counselors in matching you to a suitable pet. We make every effort to match our foster pets to appropriate homes. If you feel that we are too intrusive, please understand that it is because we care about the animals’ welfare.

The prospective caregiver agrees:

  1. Cats are to live in a private residence as companion animals.
  2. To provide the cat with sufficient quantities of nutritious food and fresh water each day.
  3. Never to strike or otherwise harm the cat.
  4. Never to have the cat declawed.
  5. Spaying or neutering is required by five weeks of age. cat is always to be spayed or neutered prior to adoption.
  6. To ensure that the cat’s vaccinations for rabies and distemper are current and to provide veterinary care upon sickness, disease or injury.
  7. To give the original caregiver visitation rights to ensure that the terms of this adoption agreement are being observed.
  8. If the cat must be relinquished for any reason by the prospective caregiver, s/he MUST NOT turn the cat over to a humane society, shelter or person, but must return the cat to the Adoptee. If the phone number and/or address for the original caregiver is no longer operative, the caregiver will make a good faith effort to locate and contact the original caregiver.
  9. I understand that failure to perform the foregoing agreement will constitute a breach of contract. In the event of any such breach of contract, I authorize the original caregiver to reclaim possession of the adopted cat.
    In return for the above conditions, the original caregiver agrees to allow the prospective caregiver to adopt the above mentioned cat.
